CITY OF GLOUCESTER, NJ -- Around 1035Hrs. Task Force 5 was transmitted to 829 Bergan Street for the
reported house fire. Squad 51 / Battalion 504 and BLS 558 were alert on the BOX assignment. Battalion 504
advised Camden County Communications he had a common in the area. Battalion 504 arrived reporting a 2
story brick house with heavy smoke and fire pushing from the house on the Charlie side. Command reported
he had people unaccounted for and ordered Squad 51 to go ins ervice with an 1 3/4 line and search for the
missing people. Command 500 reached the 10 Minute mark in the incident reporting heavy fire still in the
attic area and companies were getting water on it. Interior reported to command that the Primary and
Secondary searches were (-)Negative and they were making progress on the fire. Companies had 2 lines in
service and the ladder companies vented, The fire was marked under control @ 1103Hrs.

Working House Fire, Camden County, NJ 08.01.2007
PHOENIXVILLE BOROUGH, PA -- At 11:34 Hrs the Phoenixville Fire Department along with mutual aid from
Valley Forge and Kimberton was dispatched to Main Street and Prospect Street for a building fire. Units
arrived to find fire on the second floor of the apartment building. Crews found a kitchen fire that extended
into the attic area. As conditions inside began to deteriorate Prospect Street Command requested the 2nd
Alarm. With the second alarm bringing in the initial relocates, Rescue 4 was dispatched at 12:04 to relocate
to Station 67. Shortly after Rescue 4 responded Chester County Fire board advised Rescue 4 they were to
respond into the fire scene on the 3rd alarm assignment at 12:14 hrs. Rescue 4 arrived and the crew went to
the manpower staging area at Station 65. The crew was then advised to enter the structure on the D side
and check for fire extension. After quickly clearing the basement, first, second, and third floors, the crew
opened up an attic space and located fire begining to make its way into the attic area. The crew with help
from East Whiteland, and Valley Forge Fire Companies pulled a 1 3/4" hand line and began to hook he ceiling
and extinguish the visible fire in the attic. After the crews finished up on the inside, they raised a 35 foot
ground ladder to the roof of the building and utilized roof ladders and saws to cut two vent holes to assure
extension had been cut off. After the holes were cut the interior crew wet down the area one more time and
Battalion 4 (Johnson) advised command that the side D structure was of no hazard.

3 Alarm Apartment Building, Chester County, PA 08.01.2007
DALE CITY, VA -- Units respond to an Auto Accident with One Trapped in Co. 13's Due Just before midnight,
an ALS Auto Accident assignment was dispatched for a van into a truck with one trapped. The caller, who
was the driver of the van, reported that his legs were pinned. BC505, Engine 513 and Medic 513 arrived at
the same time and confirmed one trapped. Rescue 510 arrived swiftly and began the disentanglement
process. After the driver's door was removed, the men of Rescue 510 rammed the dash off of the victim.
Once freed, Ambulance 506 took over patient care after Medic 513 did a trauma assessment. All units
cleared shortly after.

PIC w/One Trapped, Prince William County, VA 08.01.2007
CITY OF CHESTER, PA -- Shortly after 1815hrs, Company 81, 82 for the Tower, Chester 11 and Medics were
dispatched to 9th and Kerlibn for an accident with possible entrapment.  Chester 11 arrived first and found a
2 car MVA with the drivers in both cars.  The one driver was able to get out once the traffic light pole that
was knocked down was moved but at the request of Medic 100 Tower 82 put the rescue tools in service to
remove the B post on the second car.  Once this was completed (using the combi and cutters) the crews
assisted EMS with patient packaging.  81-7, 82-7 and 83-7 transported to CCMC.  Companies were back in
quarters by 1910hrs.

PIC w/Entrapment, Delaware County, PA 08.03.2007
DRUMORE TOWNSHIP, PA -- The Rescue Local was transmitted to the area of Route 372 and Penny Road for
the reported PIC with a vehicle into a house. Units from Companies 57 and 58 arrived to find a pick up truck
left the roadway and hit the Delta Side of a 2 Story wood frame house. Command requested the Lancaster
County Collapse team be added to the assignment to help shore up the house. Companies operated for
about 3 hours, the driver of the truck was transported to LGH.

PIC w/Truck Into House, Lancaster County, PA 08.04.2007
CITY OF CHESTER, PA -- Shortly after lunch, Company 82, Chester 11, and CCMC EMS were dispatched to
22nd st and Wetherill for an PIC. Further reports were giving while the units responded that one car did
have entrapment. As Company 82 and Chester 11 arrived the reports of entrapment were confirmed and the
tower went in service with the spreaders to remove the door. The units operated just under 45 mins. On
scene were Company 82, Chester 11, Medic 100, Micu's 83-7, 100-7, 82-7.

PIC w/Entrapment, Delaware County, PA 08.05.2007
NEWTOWN SQUARE, PA -- Newtown Square Fire Company along with Marple Township EMS and Newtown and
Marple Police responded to a motor vehicle accident with entrapment and rollover. 41-9 was the first fire
officer on scene and reported  A car on its roof with entrapment. Rescue 41 responded within mins and
began extrication. Subject was flown to nearby trauma center. Crews were on scene for about 1 1/2 hours.

PIC w/One Trapped, Delaware County, PA 08.05.2007
CHESTER CITY, PA -- At 1215hrs companies were dispatched for an
MVA w/entrapment.  Subject was reported to be pinned underneath
the vehicle.  Chester 11 arrived on scene with a one vehicle
accident. The vehicle jumped the curb, crashed into the building, and
caused substantial structural damage.  One person was injured, he
was sitting on the front step of the building that was struck by the
vehicle, but was not pinned underneath.  Subject was transported to
CCMC in serious condition.

COLERAIN TOWNSHIP, PA -- The Rescue Local was transmitted to the area of Route 472 (Kirkwood Pike) just
south of Farmdale Road for the reported Truck into a horse drawn wagon. Engine 57-2 under the command
of FF J. Herr arrived on scene with an Allied Waste truck into an Amish horse drawn wagon with produce all
over the roadway. EMS and Fire personnel checked on injuries, either of the drivers were injured in the
crash. EMS was released by Command as the Engine company cleared the roadway of smashed Watermelons
and Cucumbers.

PIC Involving A Horse Drawn Wagon, Lancaster County, PA 08.09.2007
WOODBRIDGE, VA -- Rescue 510 extricates 2 from a head-on collision in Woodbridge. On Friday, August 10th
at 2205 hours, Communications dispatched Wagon and Ambulance 512 for an auto accident in the vicinity of
Jeff Davis Highway and Neabsco Mills Road. The County Police found the accident on Neabsco Mills Road
below the Northern Virginia Community College campus. They advised Communications of a head-on
collision with the front seat occupants "trapped" and the driver of the other vehicle laying in the street.
Communications upgraded the call and added BC506, Rescue 510, and Medic 517. Wagon 512 arrived on the
scene, confirmed the entrapment, and asked for an additional medic unit (Medic 552) and a helicopter
(MedSTAR 2). The crews from Wagon 512 and Rescue 510 removed the roof and rolled the dash to free the
trapped occupants. Medic 517 rapidly removed the unconscious passenger and started toward the LZ.
Engine 510 and BC505 were added to the call to handle the LZ at Freedom High School. BC506 arrived and
held the 'Neabsco Mills Road' Command. A second helicopter was added for the trapped patient. Helicopter
LifeEvac 2 arrived with MedSTAR 2 at 2240. Both patients were transported to the Trauma Service at Inova
Fairfax Hospitals. Ambulance 512 obtained a refusal from the driver of the other vehicle.

COLUMBIA BOROUGH, PA -- At approximately 01:45, Columbia Borough units were alerted to a vehicle
accident with possible entrapment in the 800 block of Lancaster Avenue across from Pizza Hut. Engine 871
went en route with 7 as Deputy 87 was arriving confirming a single vehicle accident with entrapment.
Rescue 841 arrived to start extrication on 4 patients while Engine 871 pulled a line for suppression if
needed. York Rescue 41 was added to the assignment to assist. Unknown on the extrication time.

Fatal PIC w/4 Trapped, Lancaster County, PA 08.11.2007
CITY OF CHESTER, PA -- Around 1057Hrs. The BOX was transmitted for a reported house fire. Chester 11
arrived and found a Previously burned vacant twin with an active fire in the walls and basement. Crews had  
heavy fire in the Charlie Side of the house. Engine 82, Squirt 81 and Tower 82 had 2 Deck Guns and 2  1 3/4
hand lines to handle the fire.

Working House Fire, Delaware County, PA 08.11.2007
PARKSIDE, PA -- At approximately 0200hrs companies 45(Parkside), 57(Upland), 65(Garden City), 63 and RIT 43
(Holmes) were dispatched to 11 Park Vallei Lane for a building fire. The Police, followed shortly there after by
Parkside Deputy Chief John Albany (459A) arrived and confirmed a working fire. Tac-45 arrived first and put a
line in service. Ladder 65 (Garden City) and Rescue 63 arrived almost simultaneously. At this time, Deputy
Chief Albany immediately advised the crews from Garden City and Green Ridge of a collapse with firefighters
trapped. D/C Albany requested the second alarm assignment be dispatched. Crews on the scene worked
feverously under heavy fire conditions to locate and extricate the trapped firefighters. FF Brian Harris Sr.
immediately assisted the operator from Tac 45 in securing a water supply. Conditions continued to
deteriorate. Under the command of FF Dave Evans Jr. the crew from Rescue 63 entered an exposure to gain
quicker access to side "C" of the fire building. Firefighters Evans Jr.,Thomas Morgan Jr. and Matt Didmenico
(45) entered the fire building and quickly located and freed one of the trapped firefighters from side “C” to
awaiting EMS. At this point FF Jack McPartland pulled a second handline and continued fire surpression on
the “A” side. Firefighters Dave Evans Jr., Thomas Morgan Jr. came back to side “A” to assist firefighters Jim
Ayers Sr., Jim Ayers Jr., Andrew Plotts, and Jim Carr III along with members of Ladder 65 and RIT43 worked to
move a tremendous amount of debris from the collapse in an effort to find the second trapped firefighter.
Crews gave it their all – in many cases working on their knees moving debris with their hands. Seconds
turned into minutes as crews miraculously located the second downed firefighter. He was extricated
through the front door and was met by awaiting EMS crews.
