COLLINGSWOOD, NJ -- This morning at 4:00 Task Force 16 was dispatched to the area of Route 130 and Sloan
Avenue for a report of a building fire. On arrival 1601 (Chief Amet) reported that a building was well
involved. The "All Hands" box was filled. That bought Ladder 15-1, and Cascade 11 (Pennsauken). Engine
1613 laid into the job and went in operation with multiple hand lines. Ladder 16 reported to Division "A" and
went in operation. At the twenty minute mark a second alarm was struck bringing Station 15-1 (Westmont),
Engine 1132 (Pennsauken) and Rescue 4-5 (Mt. Ephraim) for an additional FAST. Additional Engine
Companies Engine 1521 (West Collingswood Heights),Engine 1292 (Merchantville),Engine 1131
(Pennsauken),Engine 7and 10 (Camden), were all involved in water supply operations.
Collingswood was covered by Engine 211 (Haddon Heights), and Ladder 14 (Haddonfield). The fire was
placed under control at 5:45 am by Chief Amet. The Camden County Fire Marshals office is on scene to

JACKSON TOWNSHIP, PA -- At 11:56 Harmony Station 23 and Harmony EMS Station 62 was alerted for a
personal injury accident with entrapment on Interstate 79 in the area of the 88 Mile Marker southbound.
Chief 23 arrived on scene and told incoming units that there was heavy entrapment. At that time the closest
available helicopter was placed on standby. Once on scene units found a tri axle dump truck rolled over
with the driver pinned to the floor. Crew immediately went to work extricating the trapped occupant by
removing both doors and the roof. Once access was gained crews found the drivers legs pinned under the
seat. The passenger side seat was removed completely and the drivers side seat was the lifted using the
hurst spreaders. A landing zone was established on the interstate with the ground contact being Tanker 23.
The patient was flown to a Pittsburgh trauma facility with multiple traumatic injuries. Extrication time was

MANHATTAN BOROUGH, NY -- FDNY Communications transmitted BOX 0047 to 130 Liberty Street for the reported Hi-Rise building on fire. First calls reported a smell of smoke coming from the area of the building. During operations a 10-66 (Mayday) was transmitted as 2 Firefighter were found downed while operating inside the building. The firefighters were removed by fellow brothers as the Assignment reached the 5th Alarm. The 2 Downed members were rushed to local hospitals in critical condition. The fire over took many floors of the building as the 7th Alarm was transmitted and the word of the loss of our 2 Brother lost in battle was made known. FDNY communications transmitted the 5-5-5-5 for Brothers, FF Joseph Graffagnino with Ladder 5, Detailed to Engine 24 today as a fill in and FF Robert Beddia with Engine 24. Our Brothers were lost in battle, but will never be forgotten. May our Brothers Rest In Peace.... Well Done Brothers!

LIVINGSTON COUNTY, NY -- On Saturday, August 18, Hemlock and Livonia Fire/EMS responded to a fully
involved camper fire behind a residence on Route 15A in Hemlock. The fire was quickly extinguished. There
were no injuries. The cause of the fire was under investigation by the Livingston County Office of Emergency

MARTIC TOWNSHIP, PA -- Lancaster & York County Communications transmitted the Rescue Local in the area
of Route 372 & Old Pinnacle Road for the reported PIC with entrapment. D/C 58 arrived on scene confirming
the crash to be in Lancaster County and he had one patient trapped in a vehicle. Crews from Company 58,
55(York), 57, 57(York) worked along with EMS from 77 to displace the drivers door on the station wagon to
free the patient. The patient was transported to LGH by ground.

BURNHAM BOROUGH, PA -- At 1715Hrs. BOX 01-10 was transmitted for the Rescue Local in the area of North
Logan Blvd and West Freedom Avenue. Communications advised all incoming units of a 2 Vehicle PIC with
people trapped. Company 1, Engine 10-1, Rescue Squad 6-1 and Two EMS units from Company 12 arrived on
scene with a Mini van overturned with two patients trapped. Crews worked to crib the van as the doors
needed to be displaced and removed to complete extrication. Engine 1-1's Crew pulled a hand line for
protection if needed during rescue operations. Both patients from the van were rushed to Lewistown
Hospital with a third BLS unit from Company 12 requested to the scene for another patient to be transported

CHIEFTON HILL, WV -- At 03:38 hours on Sunday August 19, Companies
8 (Reynoldsville), 7 (Nutter Fort), 12 (Stonewood), 13 (Summit Park)
and 3 (Lost Creek) were alerted for a structure fire on Chiefton Hill. At
03:50 NFFD Engine 71 (w/6) and RVFD Engine 82 (w/4) were the first
arriving units and found a 2-story dwelling with heavy fire showing.
NFFD, RVFD, SWVFD and SPVFD provided interior attack crews and
knocked the bulk of the fire down fairly quickly. Exterior operations
were also performed. At that time Taylor County Co. 30 (Flemington)
was added to the alarm for tanker support and Harrison County Co. 5
(Mount Clare) was alerted for a transfer to Co. 8. After 3 tone outs
Mount Clare had not responded so Co. 4 (Lumberport) was
transferred to Co. 8. Crews performed salvage and overhaul
operations and went ready at 07:22 hours. The cause of the fire is

CITY OF PHILADELPHIA, PA -- On August 18, 2007, Tactical box 6722 was dispatched at 10:31 p.m at 6730
Bustleton Ave. First arriving units found heavy fire conditions from a 2 story 15x35 store in the middle of a
row. Crews led off with a 1 ¾” hand line. Command arrived and confirmed a working fire and made several
special calls. Also added to the box was Rescue 1 and Squad 72. Crews made an aggressive interior attack
to knock the fire down without extension. The fire was placed under control around the 30 minute mark. No

POTTSTOWN, PA -- Shortly Before 7:00PM The Pottstown Fire Dept (69) was alerted to smoke in the area in of
Keim and Cross streets. County 911 started receiving numerous calls with multiple locations do to so much
smoke. Upon arrival of the first fire officer heavy smoke was reported coming from the a metal finishing plant
and the call was upgrading to a working building fire. Within minutes of the first arriving apparatus the roof
collapsed in and heavy fire was then encountered. The Chief then ordered nobody to enter the building and
the operation then went into offensive mode. Crews remained on scene all through the nite into late
morning hours. Among the units operating on scene were.. Montgomery County Stations 69, 58, 57, 37, 59,

MITCHELLVILLE, MD -- The rescue local was transmitted to East Bound Route 50 prior to 197 for the reported
PIC with people trapped. Units arrived with one rolled over with two ejected & under the Vehicle. Rescue
Engine 18 Started extrication on the two victims as they requested another Squad to assist them. Squad 8
was alerted to assist Company 18 with lifting the van off of the patients. One victim under the front end was a
Priority 4 at the scene. Crews continued to work for over 30 minutes to free the one patient under the rear of
the van. The victim was freed as a Priority 2 truama patient. A total of 6 people were transported to a local

Fatal PIC w/Two Pinned Under Van, Prince George's County, MD 08.23.2007

LANCASTER TOWNSHIP, PA -- On August 29 around 17:10 Harmony Station 23 and Harmony EMS Station 62 was
dispatched to a motorcycle accident with injuries on Interstate 79 at the 90.6 Mile Marker northbound. Units
from Harmony EMS arrived on scene and requested 2 helicopters to the scene. Rescue 23 arrived and
assisted EMS crews with patient care and packaging. Neither of the motorcyclists were wearing helmets.
The driver sustained massive head trauma and was Priority 4 and the 2nd helicopter was cancelled. Life
Flight landed on the interstate and flew the passenger to a Pittsburgh Trauma Facility.

BOSTON, MA -- BOX 281 was transmitted to 1727 Centre Street for a reported fire at the Hi-Ho Chinese Restaurant in the West Roxbury Section. Firefighter arrived with a working fire and started to good to work. While operating a couple Firefighters operating became trapped after a collapse that involved a roof air conditioner unit. Firefighters pulled all the member out and CPR was started on two members as they were pulled out not breathing. The two Members not breathing were rushed to a local Hospital were they expired shortly after arrival.
The Two Firefighters Killed were from Engine 30 and Ladder 25. Five Other Firefighters were transported to other local hospitals with less serious injuries.

Roof Collapse At Chinese Restaurant Fire Claims Two Brothers, 08.29.2007

PRINCE FREDERICK, MD -- At 1052 hours, the Prince Frederick and Benedict Volunteers were alerted to
Rescue Box 02-05 for a three vehicle crash on Hallowing Point Road (Md.Rte 231) at Hallowing Lane in Prince
Frederick. Rescue Squad 2(Prince Frederick), Rescue Engine 83-2(Benedict), Ambulance 83-8(Benedict) and
Ambulance 6-8(Huntingtown) responded. RS-2 arrived on the scene and found one vehicle in the roadway
and a tractor trailer on top of another car on fire. Lt. Michael Sharpe of Prince Frederick established the
Hallowing Point Command. RS-2 began knocking the fire down as other units were added to the run. Three
Engines, two Squads and two tankers brought the fire under control in 15 minutes. An investigation revealed
that a 2002 Dodge Neon was stopped on west bound MD Rt. 231 preparing to make a left turn onto Hallowing
Lane. A 2004 Toyota Corolla operated by Sallie N. Harding, 60 of Hollywood, was also traveling west. Ms.
Harding failed to see the Dodge in time to avoid running into its rear. The impact pushed the Dodge Neon
across the center line and into the eastbound lane of MD Rt. 231. At that time, a 1998 Mack tractor trailer
pulling a loaded with sod ran over the Dodge Neon. The Dodge Neon became trapped under the tractor
trailer. The tractor trailer and the trapped Dodge slid into a gas station and struck a concrete island with
three fuel pumps. As both vehicles slid to a final rest, a fire erupted. The truck driver, Stephen O. Dixon, 67
of Mechanicsville, was able to exit the truck before it was engulfed in flames. The sole occupant of the
Dodge Neon could not be accessed by bystanders or the first arriving units due to the amount of fire. Ms.
Harding was able to bring her vehicle to a stop on the westbound shoulder of MD Rt. 231. Ms. Harding was
evaluated at the scene and refused treatment. Mr. Dixon was transported to the Calvert Memorial Hospital
where he was treated and released. MD Rt. 231 was closed until 1425 hours while the investigation was
conducted. Personnel from Prince Frederick Volunteer Fire Department, Huntingtown Volunteer Fire
Department and Rescue Squad, Hughesville Volunteer Fire Department and the Benedict Volunteer Fire
Department and Rescue Squad. Two fuel pumps which were damaged by this crash did not rupture, and no
fuel from the destroyed pumps was spilled. Luckily, the manager activated the emergency cut off for these
pumps seconds after the crash.

BELTSVILLE, MD -- The Rescue Local was transmitted to the area of Edmonston Road near Sunnyside Avenue
for the reported PIC with people trapped. Units arrived on scene with a trash truck head-on into a mini van
with one person trapped in the van. As crews started to go to work, the person trapped in the van was
pronounced as a Priority 4 on scene. Crews cleaned up and contained hazards as the investigation

COLLINGSWOOD, NJ -- On Saturday August 25, 2007, Engine 1613 and BLS 1647 were dispatched to the area
of White Horse Pike and Collings Ave for a reported vehicle fire. Upon arrival, crews found a vehicle heavy
involved. The Engine Company went in operation with a 1 ¾” hand line to extinguish the blaze. The fire was
being fueled by a ruptured fuel tank. Witnesses stated the vehicle ran over a man hole cover and started the

HADDONFIELD, NJ -- Around 1030 pm on Wednesday August 29, 2007 a box was dispatched in Haddonfield for
a reported dwelling fire at 73 Center Street. Police arrived and reported fire in the rear room of the dwelling
with all occupants evacuated. Chief 1401 arrived and reported a 3 story wood frame dwelling, 25x50 with a
working fire. Command struck the all hands. Crews went in operation with a 1 ¾” hand line to find heavy fire
conditions in Division C with fire extending to the 2nd floor. A 2nd 1 ¾” hand line was placed in operation to
quickly knock down the blaze. Crews opened up and checked for extension as Command placed the fire
under control. No injuries were reported on the fire ground. The Camden County Fire Marshals were called

CITY OF CAMDEN, NJ -- On Friday, August 31, 2007 around 1 p.m.,
Engine 1, Rescue 1, Battalion 1 and EMS were dispatched for a
motor vehicle accident rescue assignment at Haddon Ave and
Federal St in Camden City. Camden County Communications
reported a Bus hit a pole which brought down wires on a
vehicle. The occupant was reported to be still in the vehicle.
Upon Fire Department arrival, crews found a bus which clipped a
traffic light bringing down a pole on top of another vehicle
waiting at the light. No extrication was required. No injuries were

MANHEIM BOROUGH, PA -- BOX 26 Was transmitted to 246 South Charlotte Street for the reported house on
fire. Communications advised Chief 26-2 of multiple calls of a working fire with exposures to another house
and possible people trapped. Chief 26-2 was a block out reporting heavy smoke in the area and requested
the whole first alarm be covered. On Arrival Chief 26-2 reported one house well off with Exposures to the
Bravo side to another house and Transmitted the 2nd alarm. Members from Engine 26-2 deployed a 1 3/4
CAFS line and a Blitz gun on arrival and made a quick knock on the bulk of the fire. Crews continued to
operate as command transmitted the 3rd Alarm. Crews from all over the Lancaster County area worked
