One Heavily Trapped Lancaster County, PA 05.04.2010

PARADISE TOWNSHIP, PA -- Early on Tuesday May 4th, The Rescue Local was transmitted on Route 30 West in
Paradise Township for the reporting PIC involving multiple vehicles with people trapped. Crews arrived on
scene with a tractor trailer crash with the driver trapped in the cab. Crews worked for awhile to free the
injured driver, he was then transport to a local trauma center.

EARL TOWNSHIP, PA -- Early on Thursday May 13th, BOX Alarm 39 was transmitted to 773 Maple Grove Road
for the reported barn fire. Chief 39 along with the first alarm assignment was advised by LCWC of multiple
calls of a barn on fire in the area. Crews arrived with a large chicken house well off and already to the
ground. The Maple Grove Road command transmitted the second alarm with-in five minutes of arrival and
crews started to battle the 22,500square-foot chicken house. No chickens were in the building at the time of

Dual Entrapment York County, PA 05.09.2010

HELLAM TOWNSHIP, PA -- York County 911 dispatched the Hellam Township Fire Department for a vehicle
accident with unconscious patients at 2230 to the intersection of Kreutz Creek Road and Route 462. While
personnel were enroute to staff the Rescues, 911 upgraded the crash with double entrapment into a traffic
light pole per Hellam Township PD. Rescue 41-1, Rescue 41-2, and the Command Unit responded with 10
personnel - 3 EMT's and all being Rescue Technicians. Chief 21 arrived on scene confirming the
entrapments and advised the incoming apparatus that the roof and doors needed to go and the dash
needed to be lifted. Engine 21 arrived to start stabilization, depowerment, and supply the suppression crew.
Upon arrival of the Rescue units, 41 & 21 went to work and freed the first patient within 12 minutes of being
on scene. The 2nd patient was extricated within 14 minutes of the arrival of rescue personnel. Both Class - 1
Patients were taken to the nearest trauma center. The HTFD was available 1 hour and 23 minutes from

Fatal House Fire Mineral County, WV 05.06.2010

KEYSER, WV -- On 5/6/2010 in the early morning hours, Squad 22 was dispatched for mutual aid to Mineral
County WV for a working house fire on Keys St. Squad 22 responded with 4 Volunteers and A/C Derham. Early
reports of one subject trapped inside the residence. The first arriving engine company from 33 pulled an
attack line to make entry into the house. Chief 33 (Paitsel) had the Keys St Command. One subject was pulled
from inside the residence and EMS crews began CPR on the victim. Squad 22 arrived on the scene and was
given the assignment to check the basement area and to control utilities. The squad crew check the
basement area but found no fire or extension. The utilities were also shut off to the residence. The crew
then reported to the first floor and assisted with fire attack and overhaul operations. Squad 22 remained on

EARL TOWNSHIP, PA -- On May 20th, The rescue local was transmitted to the intersection of Rt 625 and
Weaverland Road for the reported PIC w/one trapped. LCWC advised all incoming units of a two vehicle
crash with a dump truck over turned. Crews from the Goodville Fire and Rescue Company arrived and
confirmed one trapped in a cube van. EMS Command requested the direct fly of the patient trapped. The

Vehicle Inside Of A House Lancaster County, PA 05.19.2010

EARL TOWNSHIP, PA -- On May 19th, BOX Alarm 39 was transmitted to Maple Drive for the reported house on
fire. D/C 39 reported to fire radio moments after dispatch of smoke showing from a distance. Crews on the
first alarm arrived with a vehicle fire inside a garage of a 2.5 story wood frame dwelling. Tower 39 was the
first unit to work, they spilt crews and had the fire down in minutes. No injuries were reported.

One Heavily Trapped Under Trailer Lancaster County, PA 06.01.2010

EAST COCALICO TOWNSHIP, PA -- On Tuesday June 1st, The Rescue Local was transmitted to the area of
Route 222 South at Spur Road for the reported PIC with one trapped involving a tractor trailer. Units from the
Reamstown, Denver Volunteer Fire Companies along with EMS were advised of multiple calls of one person
trapped in an SUV after going under a tractor trailer. DFC 17 arrived on scene confirming one SUV under a
trailer with one trapped and requested the direct fly of Pennstar 4. Crews from Engine Company 17 and
Rescue Squad 13 went to work to free the driver as operations sector requested Rescue Squad 115 in for
add'l manpower and equipment. Crews worked for about 30 minutes to free the driver as Pennstar 4
grounded directly on Route 222. The patient was flown to a local trauma center. Companies remained on the
scene as Police Department investigated the crash scene.

One Reported Trapped Livingston County, NY 05.22.2010

LIVINGSTON COUNTY, NY - On Saturday morning, May 22, the East Avon Fire Department and Avon EMS were
dispatched to a structure fire with the possibility of people trapped at 5847 Sutton Road in the town of Avon.
East Avon Fire Chief Tim Holtz arrived on location and reported a working fire and requested mutual aid from
the Village of Avon and Lakeville Fire Departments as well as another ambulance from Lima to the scene. It
was discovered that the occupants were able to escape from the burning home, one of which was
transported by ambulance to Strong Memorial Hospital in Rochester. Firefighters fought the stubborn fire in
the two-story house for over an hour to bring it under control. Additional firefighters from Caledonia were
called in to assist with overhaul operations. Lima and Geneseo fire departments were placed on stand-by
during in the incident. The cause of the fire was under investigation by officials from the Livingston County
Office of Emergency Management and the Livingston County Sheriff's Office.

BOROUGH OF EPHRATA, PA -- On Friday June 4th, The BOX Alarm was transmitted to the area of 529 W. Main
Street in the Borough of Ephrata for the reported building fire. Ambulance 112-1/Lt 15-1 arrived first on
location with a ford SUV well off and started making a knock on the fire with multiple dry chem's to keep the
fire from spreading to a neighboring gas station. Engine 15-2 arrived later, crews deployed a line and
knocked the remaining fire down. No injuries were reported and crews remained in service for about 15

SALISBURY TOWNSHIP, PA -- On 06.09.2010 around 12:15hours, The rescue local was transmitted to the area
of Route 340 & Cambridge Road for the reported PIC involving a horse and buggy. In total Four ambulances,
5 medics units were used to tend to the patients that were ejected from the buggy. One of the patients had

LEOLA, PA -- On Monday June 7th, 2010 the units from from
311, 31 and 24 were dispatched for a vehicle accident with
entrapment involving a tractor and trailer at 206 W. Main St.,
chief 311-2 went on the scene confirming 2 patient trapped,
Engine 31-2 went on the scene first doing the engine
company function's and Rescue/Engine 311-2 went on the
scene moments later and went to work on the heavily
damaged car, with in 10 Min. both patients were extricated
and in EMS care. Company 311 remained at the scene for
about 4 hours for the police investigation. Good work to the

Keller Bros. Ford Working Fire Lancaster County, PA 06.18.2010

LITITZ BOROUGH, PA -- On Friday June 18th, BOX Alarm 25 was transmitted to Keller Bros. Ford On South
Broad Street for the reported building on fire at around 2133hours. Police Department arrived on location
and advised communications of a working fire inside the building. Chief 25 arrived moments later confirming
fire showing from side alpha and took the command. Crews worked for about 1 hour to mark the fire under
control. The fire is under investigation with no injuries reported.

Passenger Trapped Lancaster County, PA 06.20.2010

SALISBURY TOWNSHIP, PA -- On June 20th at approx. 04:24am the Gap Fire Company along with White Horse
Fire Company was dispatched to 427 School Lane Rd for a reported vehicle into a tree. First arriving officers
reported that the passenger was trapped in the vehicle. Gap Fire Rescue deployed the Holmatra tool to free
the passenger as White Horse Fire Company tended to the driver.

LEOLA, PA -- On Thursday July 8th around 01:30 company's 311,48, and 410 where dispatched for a vehicle
accident with entrapment with roll over at 2666 W. Eby Rd, chief 311-2 went on scene with confirmed
entrapment and a vehicle on its roof. Engine 410-1 went on the scene first and began the Engine Company
functions and Rescue/Engine 311-2 and Rescue 48 went on the scene together and both went in to rescue
operation on the passenger side of the vehicle, extrication was complete in about 10 Minutes. The vehicle
was going at a high rate of speed and vehicle parts where found as far away as 300 feet from the scene.

Four Alarm Building Montgomery County, PA 07.11.2010

BOROUGH OF BRIDGEPORT, PA -- A fire tore through the CJ Robinson building located in the Contential
Business Center, 55 E. Front Street, on Sunday afternoon. More than 120 firefighters from 20 plus companies
from Montgomery, Delaware and Chester Counties operated at the scene for nearly 9 hours. Due to
deteriorating conditions of the building upon arrival, all operations were conducted from the exterior.
Numerous master streams were placed into service, containing the fire to the initial structure and
preventing the spread to a conjoined warehouse. Units were on scene until the late evening dousing hot
spots and cleaning up. The building involved was one of the last remaining structures left in the Contential
Business Center after a large fire consumed most of the complex back on May 15, 2001.

Heavy Storms Bring Multiple Alarms Lancaster County, PA 07.25.2010

MANOR TOWNSHIP, PA -- On Sunday July 25th, Lancaster County was being pounded by a late afternoon
thunder storm. Units from the East Hempfield area were already working an Apartment building and another
reported house fire in the area. at around 14:15 Hours, BOX Alarm 67 was tranmitted to 30 Summitville Court
for the reported house on fire. Units from Companies 23, 24, 66, 512, 62 & 68 were alerted and advised of a
possible working fire. Engine Company 23 along with Tower 66 arrived confiriming a working fire in two units.
Companies went to work with the rain pouring. Units worked for about one hour to mark the incident under

Working House Fire Kalamazoo County, MI 07.16.2010

COMSTOCK, MI -- In the early morning hours of July 16th, firefighters from Comstock Twp. were alerted to a
report of a house fire in the 9500blk of Daylily Lane. E923 was first to arrive and reported heavy fire showing
from in front of the residence. E911, E912 and R/E922 arrived and began to pump water to E923. Since there
were no hydrants in the area, mutual aid was requested for tankers. Richland Twp. responded with
Tanker361 and Galesburg responded with Tanker1061 and Ladder1041. The fire was brought under control in
about 40mins. Galesburg's vacuum unit was called in to vacuum out all of the burnt cellulose insulation from
the attic area. Crews remained onscene for about 6hrs doing overhaul and assisting the state fire marshall.
The homeowner was taken to the hospital for smoke inhalation and is expected to recover fully.

WARWICK TOWNSHIP, PA -- On Saturday July 24th, BOX Alarm 25 was transmitted to West Newport Road for
the reported working barn fire. LCWC advised units on the first alarm of reported hay on fire inside a barn
with the barn now reported fully involved. Crews from around the Lititz area responded and found a one
story barn well off on arrival. Command transmitted the second alarm due to the location and hot temps.
Command marked the fire under control in about 30 minutes.

SADSBURY TOWNSHIP, PA -- At 3:15pm, the Gap Fire Company assisted Kinzer Fire Company on a vehicle
accident with entrapment at 879 Strasburg Rd. This accident brought in SkyCareFlight helicopter to transport
one of the four patients to Lancaster General Hospital, the other three patients were taken to the hospital by
ground.

MANHEIM TOWNSHIP, PA -- On Sunday afternoon, BOX Alarm 211 was transmitted for the reported building on
fire on North Lime Street in Manheim Twp. Units on the first alarm were advised of a working garage fire as
heavy smoke could be seen through-out the Township and Lancaster City. Units made quick work of the fire
and held the first alarm. No injuries were reported as the fire is under investigation.
